This sheet allows easy Healthy Fitness Zone lookup: goo.gl/zSFVcW. This is designed to be used in printed form, but it could, also, easily be added to Google Classroom as an assignment and set for "Make a copy for each student". When students open the assignment each will be given their own copy and their name will appear in the filename. Teachers have access to all records conveniently either from Classroom or from Google Drive within the folder for that class.
